HCPH provides a multitude of services such as medical and dental services, community programming, and health education for the approximately 2.3 million people in unincorporated Harris County. On July 20, 2021, the Commissioners Court unanimously voted to establish ACCESS Harris County, an integrated care-coordination model that works to improve outcomes for vulnerable individuals through an interdepartmental multi-disciplinary team made up of coordinated care teams. The Community Health Worker/Eligibility Specialist is a front-line public health worker who is a trusted member of and has an unusually close understanding of the communities served by ACCESS Harris. Oversees onboarding process for all new HCPH clinical program participants from beginning to end. Under the direction of the ACCESS Harris IMDT Manager and the supervision of the cohort manager, the Community Health Worker/Eligibility Specialist is responsible for developing partnerships, scheduling appointments, determining program-participant eligibility, and preparing in advance for all patient appointments. Nurtures new and old relationships with collaborative partners. This position will primarily work out in the field, addressing the needs of the ACCESS Harris cohort participants, and at various events within Harris County. Duties & Responsibilities: Maintains a pipeline of candidates that are eligible for ACCESS Harris safety net programs Point of contact for all potential HCPH clinical program candidates and partnering safety net organizations Greets and guides patients through the ACCESS Harris registration process Maintains a calendar of outreach activities, including community events, workshops, appearances, and other clinical program-related events Prepares timely and accurate reports indicating progress toward program goals and outcomes Assists with creation of singular eligibility document in alignment with ACCESS Harris Safety Net Collaborative; ensures the completion of patient paperwork and eligibility, in accordance with HIPPA guidelines Thoroughly reviews all paperwork prior to the patient being escorted to the clinical area Collects consent forms/paperwork and maintains a list of all pre-registered patients for events Identifies additional cohort participants from pre-registered patients at events Assists with data collection and preparation of the community outreach budget and KPIs Other duties as assigned, including special tasks involved in responding to an emergency event Harris County is an Equal Opportunity Employer https://hrrm.harriscountytx.gov/Pages/EqualEmploymentOpportunityPlan.aspx If you need special services or accommodations, please call (713) 274-5445 or email . Requirements Education: High school diploma or G.E.D equivalent. Experience: At least one (1) year of clinical, medical, or public health administration experience in a community, public health, health clinic, physician's office or hospital environment.
